The short version: A number of herbal supplements can nudge the blood toward bleeding more easily, mostly by making platelets less sticky rather than by acting like warfarin. For a healthy adult who is not on a blood thinner, culinary amounts of garlic, ginger or turmeric are not a realistic hazard. The risk concentrates in three situations: strong standardized extracts, combination with prescription anticoagulants or antiplatelet drugs, and the run-up to surgery or a dental procedure. The evidence in this area is thinner than the confident advice you find online, so the honest version below labels what is well supported and what is mostly theory.

Why this matters after 50
Two things become common in the second half of life at roughly the same time. The first is antithrombotic medication: warfarin, the newer direct oral anticoagulants such as apixaban and rivaroxaban, and antiplatelet drugs including aspirin and clopidogrel, prescribed for atrial fibrillation, clots, stents and stroke prevention. The second is a supplement shelf that has quietly grown — a turmeric capsule for joints, fish oil for the heart, ginkgo for memory, garlic for cholesterol, maybe a nattokinase product a friend recommended.
Those two habits rarely get discussed in the same appointment. Surveys of older adults consistently find that a large share of supplement users never mention their products to a prescriber, often because they do not think of a plant extract as a drug. Meanwhile, the same physiology that makes anticoagulation valuable after 50 — a higher clotting risk — is accompanied by a higher bleeding risk: more fragile blood vessels, thinner skin, reduced kidney function that changes how some drugs clear, more falls, and often more than one medicine acting on the same pathway.
The point of this article is not to frighten anyone away from herbs. It is to separate the handful of combinations worth a real conversation from the long list of internet warnings with almost nothing behind them.
How "blood thinning" actually works
"Blood thinner" is a plain-English shortcut for two different biological jobs. Understanding which one an herb affects explains why some interactions matter more than others.
Anticoagulants act on clotting proteins. Blood clotting runs on a cascade of proteins, several of which the liver can only build with vitamin K. Warfarin blocks that vitamin-K recycling step, so fewer functional clotting factors get made. The newer direct oral anticoagulants work further down the chain, blocking a single specific clotting factor — thrombin in the case of dabigatran, factor Xa for apixaban, rivaroxaban and edoxaban. These drugs slow the formation of the fibrin mesh that holds a clot together.
Antiplatelet drugs act on platelets. Platelets are the small cell fragments that arrive first at an injured vessel and clump together to form the initial plug. Aspirin permanently disables an enzyme platelets need to recruit each other; clopidogrel blocks a receptor that does the same job by a different route. These drugs do not change the clotting proteins at all — they make the first stage of clotting less efficient.
Most herbs sit on the platelet side. This is the single most useful thing to know. When laboratory studies find that garlic, ginger, ginkgo, curcumin or feverfew "thin the blood," what they almost always mean is reduced platelet aggregation — the platelets clump less readily in a test tube. Only a few botanicals, notably danshen and coumarin-containing plants such as dong quai, appear to reach into the clotting-protein machinery itself, and those are the ones with the most alarming case reports.
That distinction has a practical consequence. A platelet effect added to aspirin or clopidogrel is additive on the same pathway. A platelet effect added to warfarin may not move the INR at all — which is why an INR test can look perfectly normal in someone who is nevertheless bruising more easily.
A word about evidence quality. Most of what is known here comes from three imperfect sources: laboratory and animal studies showing a mechanism, small human trials measuring platelet function or clotting times in healthy volunteers, and individual case reports describing bleeding in a person who happened to be taking a supplement. Case reports cannot prove cause. Volunteer studies rarely include the older adults on four medications who are actually at risk. And large randomized trials measuring bleeding outcomes for herbal products essentially do not exist. So the grades below describe how confident the field is, not how dangerous something is guaranteed to be.
The herbs with the most evidence
Garlic
Mechanism. Garlic's sulfur compounds, particularly allicin and its breakdown products, reduce platelet aggregation and may modestly relax blood vessels. The effect is reproducible in laboratory work and in some human platelet-function studies, and it appears to be dose-dependent — concentrated aged or standardized extracts do considerably more than a clove in a stir-fry.
Evidence strength: moderate for a platelet effect, weak for actual bleeding. Human studies fairly consistently show measurable reductions in platelet stickiness with concentrated garlic preparations. Reports of clinically significant bleeding are rare, and largely involve high-dose supplements, surgery, or combination with antiplatelet drugs. There are published cases of unexpected bleeding during operations in heavy garlic-supplement users.
Practical read. Cooking with garlic is not the issue. A standardized garlic extract taken daily alongside aspirin or clopidogrel, or in the weeks before surgery, is worth raising with a pharmacist.
Ginger
Mechanism. Gingerols and shogaols inhibit thromboxane production, one of the signals platelets use to recruit one another — mechanistically similar in direction to what aspirin does, though far weaker.
Evidence strength: mixed and genuinely uncertain. Laboratory work shows the platelet effect clearly. Human trials have been inconsistent: several studies of ordinary ginger doses found no measurable change in platelet function or in warfarin's effect, while a few found modest changes. A small number of case reports describe raised INR or bruising in people taking ginger with warfarin, but these are hard to attribute.
Practical read. Culinary ginger, ginger tea and standard nausea doses have not been shown to be a meaningful bleeding risk. High-dose concentrated extracts in someone already anticoagulated deserve a mention to the clinician, mostly because it is cheap to be careful.
Ginkgo biloba
Mechanism. Ginkgo contains ginkgolides, one of which — ginkgolide B — blocks platelet-activating factor, a potent trigger for platelet clumping. This is a specific, well-characterized mechanism rather than a vague antioxidant story.
Evidence strength: this is the botanical with the most concerning case literature. Published reports describe spontaneous bleeding inside the eye, bleeding around the brain (subdural and subarachnoid), and post-surgical bleeding in ginkgo users, sometimes without any other blood thinner involved. Against that, controlled trials in healthy volunteers, including some fairly large ones, have often failed to show meaningful changes in bleeding time or platelet function, and a systematic review of trials did not find a clear increase in bleeding events.
Practical read. The mismatch between alarming case reports and reassuring trials is exactly why ginkgo is on almost every pre-surgical "stop this" list despite imperfect proof. Given that the evidence for ginkgo's cognitive benefit is itself weak, the risk-benefit calculation for someone on an anticoagulant is not favorable. Discuss before combining.
Turmeric and curcumin
Mechanism. Curcumin inhibits several inflammatory enzymes, including the thromboxane pathway that platelets rely on, and shows anticoagulant activity in laboratory models. Modern supplements complicate the picture: curcumin is poorly absorbed on its own, so many products add piperine from black pepper or use phospholipid and nanoparticle formulations that raise blood levels dramatically above what any amount of curry could achieve.
Evidence strength: weak to moderate, and formulation-dependent. The mechanism is well described in laboratory studies. Human bleeding evidence is limited to scattered case reports, several involving high-absorption products combined with warfarin or antiplatelet drugs. There is no good trial evidence quantifying the risk.
Practical read. Turmeric as a spice is not the concern. A high-bioavailability curcumin supplement, particularly at the multi-gram doses used for joint pain, is a reasonable thing to disclose if you take an anticoagulant or antiplatelet drug or have surgery planned.
Fish oil and omega-3s
Mechanism. EPA and DHA are incorporated into platelet membranes and shift the balance of signalling molecules toward less aggregation. This is real, measurable, and dose-related.
Evidence strength: mechanism strong, clinical bleeding risk weaker than its reputation. This is the area where the popular understanding has drifted furthest from the data. Systematic reviews and pooled analyses of surgical and cardiology patients have generally not found that omega-3 supplementation causes clinically important bleeding, including in people taking aspirin or anticoagulants, and some surgical guidance has moved away from routinely stopping fish oil before procedures. Very high prescription-strength doses, and the specific setting of a patient with multiple bleeding risk factors, remain a genuine clinical conversation.
Practical read. Do not panic about a standard fish-oil capsule. Do tell the prescriber the actual dose if you take a concentrated high-EPA product, and let the surgical team decide about the pre-procedure window.
Vitamin E
Mechanism. At high doses, alpha-tocopherol inhibits platelet aggregation and can interfere with vitamin K–dependent clotting factor activity — meaning it has a foot in both camps.
Evidence strength: moderate, and clearly dose-dependent. Trial data at high supplemental doses have shown increased risk of certain bleeding events, most notably a signal for hemorrhagic stroke in one large prevention trial. Ordinary multivitamin-level intake has no such signal.
Practical read. High-dose vitamin E — hundreds of international units daily — is one of the more legitimate concerns on this list, particularly alongside warfarin or aspirin. There is also very little evidence of benefit at those doses, which makes it an easy thing to reconsider.
Dong quai
Mechanism. Dong quai contains natural coumarin derivatives — chemically related to the family that warfarin came from — plus compounds with antiplatelet activity.
Evidence strength: limited human data, but a plausible and serious mechanism. The most cited evidence is a case report of a marked, unexplained rise in INR in a woman on stable warfarin who had started dong quai, with the INR returning toward normal after she stopped. Systematic evidence is scant.
Practical read. The combination of coumarin chemistry and warfarin is the kind of theoretical risk worth respecting even without a trial. Treat dong quai as a "check first" herb for anyone on an anticoagulant.
Danshen
Mechanism. Danshen, the root of Salvia miltiorrhiza used in traditional Chinese medicine for circulatory complaints, appears to act on several fronts: reduced platelet aggregation, interference with clotting factors, and effects on how warfarin itself is cleared from the body.
Evidence strength: among the strongest interaction signals of any botanical here. Multiple case reports describe substantially raised INR and bleeding in patients combining danshen with warfarin, supported by animal pharmacokinetic studies showing danshen increases warfarin exposure.
Practical read. This is one of the few herbs where "do not combine with warfarin without specialist supervision" is a fair summary rather than an overreaction.
Nattokinase
Mechanism. Nattokinase is an enzyme derived from natto, fermented soybeans. Unlike the platelet-effect herbs, it appears to break down fibrin — the protein mesh of a clot — which is closer to what clot-dissolving drugs do than to what aspirin does.
Evidence strength: limited but mechanistically distinct. Small human studies show measurable changes in fibrinolytic markers. Case reports include bleeding events, including at least one report of bleeding inside the brain in a person combining nattokinase with an anticoagulant. Long-term safety data in older adults are sparse.
Practical read. Because the mechanism is different from most herbs, its effect will not necessarily show up on an INR test. Marketed heavily for circulation, it deserves more caution than its gentle "fermented food" framing suggests, especially alongside prescription anticoagulants.
Feverfew
Mechanism. Parthenolide, feverfew's main active compound, inhibits platelet aggregation and serotonin release from platelets — the same properties behind its use for migraine prevention.
Evidence strength: weak. The platelet effect is documented in laboratory studies; clinical bleeding reports are rare. The theoretical concern is additive effect with antiplatelet drugs.
Practical read. Low-priority on its own, worth mentioning if you take feverfew daily for migraines alongside aspirin or clopidogrel.
Chamomile
Mechanism. Chamomile contains natural coumarin compounds, which is the origin of the warning. Coumarins in plants are not the same thing as warfarin, and most do not have anticoagulant activity without further chemical change.
Evidence strength: very weak. The warning rests largely on a single case report of internal bleeding in a warfarin patient using chamomile products, plus the theoretical coumarin argument. Ordinary chamomile tea has no meaningful body of evidence linking it to bleeding.
Practical read. A cup of chamomile tea is not a realistic hazard. Concentrated chamomile extracts in a warfarin patient are worth a mention, and that is about as far as the evidence goes.
Pomegranate and guava leaf extracts
Mechanism. Both have been shown in laboratory work to reduce platelet aggregation. Pomegranate additionally appears to inhibit certain liver enzymes involved in drug metabolism, which could in principle raise levels of some medicines, including warfarin.
Evidence strength: preliminary. These are among the newer additions to bleeding-risk lists, based mostly on in-vitro and animal data plus a small number of case reports. Human evidence at the level of juice or ordinary supplement doses is thin.
Practical read. Not a reason to avoid pomegranate juice. Concentrated extract supplements taken daily alongside warfarin are an unknown worth flagging rather than a documented hazard.
Interactions with common blood thinners
The interaction depends as much on the drug as on the herb.
Warfarin is the most interaction-prone of the group, for two reasons. Its effect is measurable and narrow — small shifts change the INR — and it is metabolized by liver enzymes that many botanicals influence. Herbs can raise warfarin's effect by adding their own anticoagulant activity, by slowing warfarin's clearance, or by displacing it from carrier proteins. They can also reduce its effect: St. John's wort speeds warfarin's breakdown, and vitamin K–containing products directly oppose it, which risks clotting rather than bleeding. The reassuring part is that warfarin patients are monitored, so a shift often shows up as an INR change before it shows up as a problem — provided the clinic knows to look.
Direct oral anticoagulants — apixaban, rivaroxaban, edoxaban, dabigatran — are frequently described as interaction-free. They are not; they are unmonitored. There is no routine INR, so a supplement that raises drug levels produces no warning number at all. These drugs are affected mainly by substances that influence the P-glycoprotein transporter and the CYP3A4 enzyme, and several botanicals do exactly that. St. John's wort is the clearest problem, reducing DOAC levels enough to risk treatment failure. Herbs with their own antiplatelet activity add bleeding risk on a separate pathway that no blood test will flag.
Antiplatelet drugs — aspirin, clopidogrel, prasugrel, ticagrelor — line up on the same mechanism as most of the herbs discussed here. Adding a concentrated garlic, ginkgo or curcumin product to aspirin means two agents pushing the same lever. Dual antiplatelet therapy after a stent, or an antiplatelet plus an anticoagulant, already carries elevated bleeding risk; a botanical layered on top is a genuine additive concern rather than a theoretical one.
A pattern worth noticing: the highest-risk scenario is rarely a single herb. It is stacking — fish oil plus turmeric plus garlic plus a daily aspirin — where no individual component looks alarming but the combined push on platelet function is meaningful, and nobody has counted.
Warning signs of excessive bleeding
Learn these, and take them seriously if you are on any antithrombotic drug or combination.
Contact your clinician promptly for:
- Bruises appearing with no remembered injury, or bruises that keep spreading
- Gums that bleed when brushing, or nosebleeds that are new, frequent, or take more than ten to fifteen minutes to stop
- Small cuts that ooze far longer than they used to
- Pink, red, or tea-coloured urine
- Unusually heavy or prolonged menstrual bleeding
- New, unexplained fatigue, breathlessness on exertion, or pallor — possible signs of slow blood loss and anemia
Seek emergency care immediately for:
- Black, tarry, or bloody stools
- Vomiting blood or material resembling coffee grounds
- Coughing up blood
- Sudden severe headache, unlike your usual headaches
- New confusion, slurred speech, weakness or numbness on one side, or vision change
- Any head injury while taking a blood thinner, even if you feel fine — bleeding inside the skull can be delayed
- Bleeding that will not stop with firm pressure
Do not stop a prescribed anticoagulant while you wait for advice. Call and describe what is happening; the clinician decides what pauses.
Special situations
Before surgery and dental work
This is where herbal bleeding risk becomes concrete. Anesthesia and surgery bodies have long advised disclosing all herbal products before a procedure, and many institutions ask patients to stop them roughly one to two weeks beforehand — a window based on caution and pharmacology rather than on trial evidence for each product.
What matters practically:
- Disclose everything, including teas, gummies, greens powders and "natural" sleep or joint products. Surgical teams routinely find these are omitted from medication lists.
- Let the team set the timing. The right interval differs by product, dose and procedure. A cataract operation, a dental extraction and a hip replacement are not the same bleeding problem.
- Dental work counts. Extractions and deep cleanings bleed, and dentists often are not told about supplements at all.
- Do not pause a prescribed anticoagulant on your own initiative. For many minor procedures the safest course is to continue it; that decision belongs to the prescriber and the proceduralist together.
Keeping vitamin K consistent on warfarin
Warfarin's dose is calibrated against your habitual vitamin K intake. That leads to advice that surprises people: the goal is steadiness, not avoidance. Someone who eats a spinach salad most days can be perfectly well managed on warfarin — as long as they keep doing it.
The disruptions to watch for are changes: starting a green-vegetable diet, quitting one, beginning a greens powder or multivitamin containing vitamin K, a course of antibiotics that alters gut bacteria producing vitamin K, or a stretch of poor appetite during illness. Tell the anticoagulation clinic before or as soon as any of those happen so INR monitoring can be adjusted. This applies to warfarin only; direct oral anticoagulants are not affected by dietary vitamin K.
Practical safety guidance
- Ask before you combine. One conversation with a pharmacist, who can see your full medication list, catches more than any article. Pharmacists are free to consult and are the most underused resource in this whole problem.
- Never stop a prescribed blood thinner without medical advice. The clot risk that led to the prescription does not pause while you experiment. Abrupt discontinuation has caused strokes and heart attacks.
- Bring the whole list to every appointment. Not just prescriptions: vitamins, herbs, teas, powders, gummies, topical products, and anything a family member recommended. A photo of each label on your phone works as well as a written list.
- Count the stack, not the single product. Three mild platelet effects plus a daily aspirin is a different situation from any one of them alone.
- Prefer standardized products with transparent doses and third-party testing. Herbal potency varies enormously between brands and batches, which is itself a safety issue when the compound has pharmacological activity.
- Introduce one thing at a time. If something does change how easily you bruise, you want to know what caused it.
- Re-review after every change — a new prescription, a dose change, a new supplement, a planned procedure.
Bottom line
Some herbal supplements genuinely affect how readily blood clots, mostly by making platelets less sticky. The evidence is strongest, or at least most concerning, for ginkgo, danshen, dong quai, high-dose vitamin E, nattokinase and concentrated garlic. It is weaker and more formulation-dependent for ginger, turmeric, feverfew, chamomile and pomegranate. Fish oil's reputation for causing bleeding outruns what the data support at ordinary doses.
For adults over 50 on warfarin, a direct oral anticoagulant, or an antiplatelet drug — and for anyone with surgery or dental work ahead — the practical response is not to abandon herbs in a panic. It is to make the invisible visible: one complete list, one pharmacist review, honest disclosure to the surgical team, and knowing which bleeding signs mean call today and which mean call an ambulance.

What is largely missing across the whole topic are adequately powered randomized trials with bleeding as a measured outcome for individual botanical products. That absence is why this article grades confidence in words rather than numbers, and why no specific study counts, effect sizes or citation numbers are asserted here.
