When Medicine Changes Your Nutrient Needs After 50
Common prescriptions can affect vitamin B12, magnesium, potassium, and other nutrients—but that does not mean everyone needs another pill.

The short answer: Some medicines can reduce nutrient absorption, change how quickly nutrients leave the body, or interact directly with supplements. The responsible response is not to stop a prescription or automatically buy supplements. It is to identify the specific risk, look for symptoms, review your full medication list, and test when testing is useful.
Many adults over 50 take more than one prescription. That can be lifesaving. Blood-pressure medicines prevent strokes. Metformin helps control diabetes. Acid-reducing medicines can prevent ulcers and treat severe reflux.
But medication and nutrition do not occupy separate worlds. A drug can sometimes alter how a nutrient is absorbed, stored, used, or excreted. Meanwhile, a supplement can change how a medicine works.
This article separates three different questions that are too often mixed together:
- What is well established?
- What is plausible but still uncertain?
- What should a person actually do?
1. Metformin and vitamin B12: a real, recognized connection
Metformin is a first-line medication for type 2 diabetes. It is effective, inexpensive, and has a long safety record. It can also lower vitamin B12 levels in some people.
This is not an internet theory. U.S. prescribing information reports that about 7% of participants with previously normal B12 levels developed subnormal levels during 29-week metformin trials. The label recommends annual blood-count testing and checking vitamin B12 every two to three years, with abnormalities managed when found.
Vitamin B12 deficiency can cause fatigue, anemia, a sore tongue, numbness, tingling, balance trouble, or cognitive changes. Neurologic symptoms can occur even without anemia. Diabetes itself can also cause nerve symptoms, which means a treatable B12 problem could be mistaken for diabetic neuropathy.
Evidence level: Strong. Metformin can lower B12, but not every metformin user becomes deficient.
Sensible action: Do not stop metformin. Ask whether B12 testing is appropriate—particularly after long-term use or when unexplained anemia, fatigue, numbness, or tingling develops. Supplementation makes the most sense when intake is inadequate, laboratory results are low, or a clinician identifies a meaningful risk.
2. Acid reducers: B12 and magnesium deserve attention, not panic
Proton-pump inhibitors, or PPIs, include medicines such as omeprazole, esomeprazole, lansoprazole, and pantoprazole. They reduce stomach acid and can be extremely useful. Long-term use, however, has been associated with two nutrient concerns.
Vitamin B12
Stomach acid helps release food-bound B12 so the body can absorb it. NIH identifies prolonged PPI use as one possible cause of B12 deficiency. The risk is not the same for everyone, and an association does not prove that every long-term user needs a supplement.
Magnesium
Long-term PPI use has occasionally been linked to low blood magnesium. NIH notes that PPIs can affect magnesium status. Low magnesium can produce weakness, muscle cramps, tremors, abnormal heart rhythms, or seizures when severe—but these symptoms have many possible causes.
Evidence level: Moderate to strong for an association; individual risk varies.
Sensible action: Do not discontinue a PPI suddenly on the basis of an article. Ask the prescriber whether the medicine and dose are still necessary and whether B12 or magnesium testing makes sense. A supplement is not always the complete solution; in some reported cases of PPI-associated low magnesium, medical management required changing the medication.
3. Diuretics can move magnesium and potassium in different directions
“Water pill” is not one drug. Different diuretics affect minerals differently.
- Loop and thiazide diuretics can increase urinary magnesium loss. They may also lower potassium.
- Potassium-sparing diuretics can reduce magnesium loss and can raise potassium.
- ACE inhibitors and angiotensin-receptor blockers can also raise potassium, especially in people with impaired kidney function or when combined with potassium supplements or salt substitutes.
That is why “take magnesium and potassium with your blood-pressure medicine” is unsafe as blanket advice. The correct answer depends on the exact medicine, kidney function, dose, other drugs, diet, and laboratory results.
Evidence level: Strong that these medicines can alter electrolyte balance; weak support for routine self-supplementation without testing.
Sensible action: Know the exact name of your medication. Do not add potassium pills—or potassium-based salt substitutes—without checking with a clinician or pharmacist. Magnesium supplements can also interact with medicines and can be risky when kidney function is reduced.
4. Statins and CoQ10: a believable theory that has not become a dependable treatment
Statins interfere with a biochemical pathway involved in both cholesterol and coenzyme Q10 production. That creates a reasonable hypothesis: perhaps lower CoQ10 contributes to statin-related muscle symptoms, and perhaps replacing it will help.
The mechanism is plausible. The clinical results are inconsistent.
The National Center for Complementary and Integrative Health concludes that the overall evidence does not support CoQ10 as a reliable treatment for statin-associated muscle pain. Some individual trials report improvement, while others do not. Differences in statin dose, baseline CoQ10 status, supplement quality, and the true cause of pain may partly explain the conflicting results—but that explanation is still theoretical.
Evidence level: Plausible mechanism; insufficient evidence for a dependable benefit.
Sensible action: New muscle pain while taking a statin deserves a medical review. Do not quietly replace or stop a statin with CoQ10. A clinician can consider the statin, dose, interactions, thyroid function, exercise, and other possible causes.
5. Sometimes the supplement changes the medicine—not the other way around
The most important nutrient issue may be an interaction rather than a deficiency.
Warfarin and vitamin K
Warfarin works by interfering with vitamin K–dependent clotting. A sudden increase or decrease in vitamin K can change its effect and create a risk of clotting or bleeding. The goal is usually consistency, not eliminating every vitamin K–containing food.
Multivitamins, greens powders, and “bone health” products can contain vitamin K. Changing brands or doses can matter.
Magnesium, calcium, iron, and medication absorption
Minerals can bind to certain medicines in the digestive tract and reduce their absorption. Examples include some antibiotics, thyroid medicine, and osteoporosis drugs. The solution may be dose separation rather than abandoning the nutrient, but the correct interval depends on the specific medicine.
St. John’s wort
“Natural” does not mean passive. St. John’s wort can speed the breakdown of many medicines, potentially making antidepressants, heart drugs, transplant medicines, and other prescriptions less effective. It also has important interaction risks beyond nutrient status.
Evidence level: Strong for several clinically important interactions.
Sensible action: Give the pharmacist the complete list—including vitamins, powders, gummies, herbal products, teas, and occasional supplements.
The theory: “nutrient friction” may be more useful than a one-drug, one-deficiency model
Here is a hypothesis worth studying, not a proven diagnosis.
A single medicine may produce only a small change in nutrient status. But after 50, several pressures can overlap:
- reduced appetite or a less varied diet;
- age-related changes in absorption;
- two or more medicines affecting the same nutrient;
- chronic disease;
- reduced kidney function;
- alcohol use;
- over-the-counter acid reducers that never appear on the prescription list.
We call this combined burden nutrient friction: no single factor guarantees deficiency, but several modest factors may reduce the body’s margin for error.
This framework leads to a testable idea: medication reviews may work better when they examine clusters of risk rather than one prescription at a time. For example, an older adult taking metformin and a PPI, eating little animal protein, and reporting numbness has more reason for a B12 evaluation than someone with only one of those factors.
The theory should not be turned into a sales pitch. It does not prove that broad blood panels or large supplement stacks improve health. It suggests that targeted questions and selective testing could identify the people most likely to benefit.
A second theory: some “side effects of aging” may be reversible contributors
Fatigue, weakness, cramps, memory complaints, and tingling are often blamed on aging. They can also arise from medication effects, nutrient deficiencies, illness, poor sleep, inactivity, or several factors together.
It is reasonable to theorize that identifying a genuine deficiency could improve symptoms that had been accepted as inevitable aging. That does not mean every symptom is nutritional. The honest sequence is:
- identify the symptom;
- review medicines and diet;
- test when appropriate;
- treat the confirmed or strongly suspected problem;
- reassess whether the treatment helped.
This is slower than buying a “50+” supplement bundle. It is also more trustworthy.

Before buying a supplement, write down:
- Every prescription and over-the-counter medicine you take.
- Every vitamin, mineral, herb, powder, gummy, and fortified drink you use.
- How long you have taken each one.
- Any new fatigue, weakness, cramps, numbness, tingling, balance problems, or digestive changes.
- Your latest kidney-function results, if known.
- The exact nutrient you are considering and the reason.
Then ask a pharmacist or clinician:
- Could one of my medicines alter this nutrient?
- Could this supplement alter one of my medicines?
- Would a food-first approach be adequate?
- Is a laboratory test useful before supplementing?
- What dose and duration make sense if treatment is needed?
- When should the result be rechecked?
Bottom line
Some prescriptions genuinely change nutrient needs. Metformin and vitamin B12 are the clearest example. Long-term acid suppression can matter for B12 and magnesium. Diuretics can change magnesium and potassium, but the direction and danger depend on the exact drug. The statin–CoQ10 theory remains biologically plausible without reliable proof of symptom relief.
The safest principle is not “take more supplements.” It is match the intervention to the evidence:
- Strong evidence plus confirmed need: act with professional guidance.
- Plausible theory without confirmation: investigate before buying.
- Known interaction risk: protect the medicine first.
Your medication list may contain clues about nutrition. It should never become permission to stop treatment or build an unsupervised supplement stack.
