What EPA and DHA actually are
Fish oil is a delivery vehicle. The active components are two long-chain omega-3 fatty acids: eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). They are incorporated into cell membranes and converted into signalling molecules that influence inflammation, platelet behaviour, and blood-vessel tone. DHA is also concentrated in the retina and in brain grey matter, which is why it dominates discussions of vision and neurodevelopment.
The body can convert the plant omega-3 ALA (from flax, chia, and walnuts) into EPA and DHA, but the conversion rate is low and variable. That is why oily fish, or a fish or algae oil supplement, is the practical route to a meaningful intake.
Why total fish-oil milligrams are misleading
A bottle advertising “1,200 mg fish oil” is telling you the weight of the oil, not the dose of the thing that works. That softgel might contain only 300–360 mg of combined EPA and DHA. A “triple strength” product may deliver 900 mg or more from a similar-sized capsule.
Turn the bottle over and find the line that reads EPA and DHA in milligrams, then add them. Divide the price of the bottle by the total grams of EPA + DHA it contains, and you have the only price comparison that means anything in this category.
Triglycerides, and the prescription difference
The best-replicated effect of omega-3s is lowering elevated triglycerides, and the effect is dose dependent. At the pharmacological level — around 4 g per day of EPA, or EPA plus DHA, in a prescription product — triglyceride reductions of roughly 20–30% are commonly reported, with larger reductions when baseline levels are very high.
This matters for how you shop. Prescription omega-3 medicines are regulated products with verified content, approved for treating severe hypertriglyceridaemia. Ordinary over-the-counter fish oil is not a substitute for them. If your triglycerides are high enough to need treatment, that is a conversation with a clinician, not a purchase decision.
What the benefit evidence supports — and where it wobbles
Cardiovascular. Beyond triglycerides, results are genuinely mixed. Some large trials of high-dose EPA in high-risk patients showed reduced cardiovascular events; other large trials of ordinary-dose omega-3 in broadly healthy populations found little or no benefit. The honest summary is that fish oil is not established as a general heart-attack preventive for people who are not high risk. Eating fish remains the better-supported recommendation.
Brain and mood. EPA-weighted supplementation has shown modest benefits as an add-on in depression in some meta-analyses, with considerable heterogeneity. For preventing cognitive decline or dementia in healthy older adults, trials have largely been disappointing.
Eyes. DHA is structurally central to the retina, and omega-3 intake is associated with lower dry-eye symptoms in some studies, though a major randomised trial of supplements for dry eye found no advantage over placebo. Supplementation did not slow progression of age-related macular degeneration in the large AREDS2 trial.
Joints and inflammation. This is one of the more consistent areas outside triglycerides: in rheumatoid arthritis, higher-dose omega-3 has been associated with reduced joint tenderness and reduced reliance on NSAIDs in several trials. Effects on ordinary osteoarthritis or general aches are less convincing.
Pregnancy. Adequate DHA intake during pregnancy is broadly recommended, and supplementation has been linked to a reduced risk of early preterm birth in pooled analyses. Prenatal dosing should be discussed with a maternity care provider.
Risks and side effects
- GI upset and fishy aftertaste. The most common complaints. Taking the dose with a meal, freezing softgels, or choosing an enteric-coated product usually helps.
- Anticoagulant and antiplatelet interaction. Omega-3s can modestly affect platelet aggregation. If you take warfarin, a direct oral anticoagulant, clopidogrel, or regular aspirin, discuss fish oil with your clinician, and mention it before any surgery or dental procedure.
- Fish and shellfish allergy. Fish oil is derived from fish. If you have a fish allergy, do not assume purification removes the risk; algae-derived omega-3 is the safer route and should still be cleared with your clinician.
- High-dose use. Doses well above 3 g per day of combined EPA and DHA from supplements are generally considered a medical decision rather than a self-care one.
- Atrial fibrillation signal. Several high-dose omega-3 trials have reported a small but repeated increase in newly detected atrial fibrillation. The absolute risk appears low, but it is a real signal, and it argues against casually taking very large doses without a clinical reason.
- Oxidation and rancidity. Degraded oil tastes and smells sharply fishy and delivers oxidation products you do not want. Freshness handling is a genuine quality differentiator between brands.
- Contaminants. Reputable oils are distilled to remove mercury, dioxins, and PCBs, but that purification is only as good as the manufacturer's testing programme.

- Third-party testing. Look for USP, NSF, IFOS, or a manufacturer that publishes per-lot certificates of analysis.
- Oxidation and freshness. Peroxide value and TOTOX data, an unexpired date, and opaque packaging all matter more than brand prestige.
- Contaminant testing. Heavy metals, dioxins, and PCBs should be explicitly tested for and reported.
- Form. Triglyceride and re-esterified triglyceride forms are absorbed somewhat better than ethyl esters, especially on a low-fat stomach; ethyl esters taken with a proper meal are still fine for most people.
