A small number of pairings are genuinely dangerous, and a larger group can quietly reduce a medicine's effect or add to its side effects. St. John's wort: the most important herbal interaction to know

St. John's wort is marketed for mood, and it is genuinely active — which is exactly the problem. It switches on liver enzymes and drug transporters that break down medicines, so it can make a long list of prescriptions less effective: antidepressants, hormonal birth control, transplant anti-rejection drugs, HIV medicines, and some heart and cancer medicines among them. For drugs where maintaining a level is critical — a transplant drug, an HIV regimen — losing effectiveness can be serious.

The second risk runs the other direction. Combined with serotonergic medicines such as SSRI or SNRI antidepressants, St. John's wort can push serotonin activity too high, producing serotonin toxicity — agitation, sweating, rapid heart rate, and in severe cases a medical emergency.

Evidence: strong. This is a well-established interaction, not a theory. If you take any regular prescription, St. John's wort is a "check with the prescriber first" herb, full stop.

Vitamin K and warfarin: consistency, not elimination

Warfarin works by interfering with vitamin K–dependent clotting. Because of that, vitamin K intake and warfarin dose are calibrated against each other. A sudden change in either direction — starting a greens powder, quitting leafy vegetables, adding a multivitamin with vitamin K — can shift the drug's effect and create clotting or bleeding risk.

The practical guidance surprises people: the goal is usually consistency, not avoidance. Do not suddenly stop eating vitamin-K foods, and do not start or stop vitamin-K-containing supplements, without telling the anticoagulation team that monitors your INR. Evidence: strong.

Minerals that block absorption: calcium, iron, magnesium, zinc

Minerals can bind certain medicines in the digestive tract and keep them from being absorbed. The classic examples:

  • Levothyroxine (thyroid hormone) — absorption falls when taken with calcium or iron.
  • Certain antibiotics — some are inactivated by mineral binding.
  • Bisphosphonates (osteoporosis medicines) — notoriously sensitive to anything else in the stomach.

The solution is usually spacing, not abandoning the nutrient. But there is no single universal interval — the correct separation depends on the specific drug. Follow the medication label, or ask the pharmacist, rather than applying a rule of thumb you read online. Evidence: strong that binding occurs; the practical fix is drug-specific.

Bleeding-related combinations: ginkgo, garlic, vitamin E, turmeric, fish oil

Several popular supplements are discussed alongside anticoagulants and antiplatelet drugs: ginkgo, garlic, high-dose vitamin E, turmeric/curcumin, and concentrated omega-3 or fish-oil products. The concern is an additive effect on bleeding tendency.

Here the honest label is variable and often uncertain. The evidence ranges from case reports and small studies to plausible mechanisms, and the magnitude of any effect depends heavily on dose. Fish oil deserves special care in how it is described: ordinary dietary omega-3 intake has not been shown to cause clinically important bleeding in most people, and headlines often overstate the risk. What actually matters is the combination — a high-dose, concentrated omega-3 product layered on top of an anticoagulant or antiplatelet, in a person with other bleeding risks. That is a real conversation to have with a prescriber — and a very different thing from panicking over a serving of salmon.

Sedating supplements: melatonin, valerian, kava

Melatonin, valerian, kava, and similar calming products become a different proposition next to other sedatives: benzodiazepines, opioids, prescription sleep medicines, sedating antihistamines, muscle relaxants, cannabis, or alcohol. The effects add up — more drowsiness, more falls, impaired driving, and, when opioids or strong sedatives are involved, slowed breathing. After 50, fall risk alone makes this worth taking seriously.

Kava carries an additional, separate concern: it has been linked to rare but serious liver injury, which is reason enough to avoid it or to use it only with clinician oversight. Evidence: additive sedation is well established in principle; the size of the effect varies by combination and dose.

Potassium supplements and salt substitutes

Potassium is essential — and dangerous in excess. ACE inhibitors, ARBs, and potassium-sparing diuretics all tend to raise blood potassium, and reduced kidney function raises it further. That puts it on a collision course with blood-pressure medicines, diuretics, digoxin, corticosteroids, and any medicine sensitive to potassium levels. Blood pressure climbing and potassium falling together is a heart-rhythm concern.

Note that "DGL" (deglycyrrhizinated licorice) products have the glycyrrhizin removed — but product labels vary, so check what you actually have. Evidence: well established for glycyrrhizin-containing products.

Berberine and blood-sugar medicines

Berberine is popular for blood-sugar support, and that is precisely where the interaction lives: combined with glucose-lowering drugs it may push blood sugar too low, and it may also affect how the body metabolizes some other medicines. The evidence here is incomplete — plausible mechanisms and limited studies rather than settled science. The cautious reading: do not layer berberine onto diabetes medication without the prescriber monitoring your blood sugar, and treat broader interaction claims as unsettled.

Supplements around surgery: tell the team everything

Surgery concentrates every interaction risk at once: bleeding tendency, sedation, blood-pressure and blood-sugar swings, all meeting anesthesia. The correct move is disclosure, not improvisation. Tell the surgical team everything you take — vitamins, herbs, powders, gummies, occasional products included — and let them decide what should pause and when.

Do not apply a blanket self-directed "stop everything X days before surgery" rule. Whether a product matters, and how far ahead it should stop, depends on the product, the dose, and the procedure. The team's instructions beat any internet schedule.

The brown-bag medication review: the single most useful step
